    me too...typed the language in the consent letter and attached the scanned copy of the last page of the appointment letter duly filled along with signature !!

    I typed and after filling up, scanned that consent part too. means I did both since it wasn't clear

    so no need for service agreement to be scanned nd sent....?

    it is not clear to me still....opt 1 is typing d content on ms word with every thing filled up bt how wil u sign den??

    a simple mail declaring your consent will suffice the purpose
